| September 4 - 5, 2006: National Museum, Václavské náměstí 68, Prague 1. National Museum is
situated on the top of the Wenceslas square in the centre of Prague. In the close vicinity, there is a
metro station "Museum" (lines A and C). To the historical centre of the town you can get easily by

| REGISTRATION |
| Registration is possible on-line or by sending in the registration form by mail or fax. 1. For on-line registration you can fill out the electronic registration form on the conference website. 2. Alternatively: you can mail or fax the completed registration form (PDF) to the conference secretariat. Your data inserted in on–line forms are protected (Thawte certificate). Payment of the early registration fee must be received by May 31, 2006. Registration forms received without appropriate remittance will not be accepted as an early registration. A receipt will be mailed to those who have forwarded the registration fee, as confirmation of registration. Inscription of your credit card data through the internet is SSL certified. To qualify for the discounted early registration fee, a completed Registration form (including payment) must be received by May 31, 2006 by the workshop secretariat. A Registration form received without appropriate remittance will not be accepted as an early registration. Registration is only valid after receipt of the registration fee. Confirmation of your registration will be sent upon receipt your registration fee. |
| PAYMENT |
| Payment in EUR should be made in one of the following ways: 1. By bank draft, specifying the name of the sender, to the account of Conference Partners Prague Ltd., account number: 176970780/0300 or IBAN Number: CZ64 0300 0000 0001 7697 0780, Československá obchodní banka a.s., Anglicka 20, Prague 2, BIC code: CEKOCZPP 2. By credit card (VISA, Eurocard/MasterCard), providing the necessary information indicated in the appropriate spaces in the Registration form. If money is transferred by bank draft, a copy of the bank order must be sent to the Workshop secretariat. Please note that the registrant must take care of all bank charges for the bank draft. An official receipt of all payments will be a part of the workshop kit. |
| CANCELLATION OF REGISTRATION AND REFUNDS |
| Any cancellation must be notified in writing to the Workshop secretariat. The registration fee will be
refunded after the deduction of 30 % if participation is cancelled prior to July 5, 2006. No refund will
be possible after this date but complete documentation will be sent to the address of the delegate. |
| REFRESHMENT AND LUNCHEONS |
| Refreshments and lunches are included in the registration fee. Refreshment during the excursion or
trip will be provided by lunch packets. |
| CLIMATE AND DRESS CODE |
| The weather at the beginning of September is usually nice but unpredictable. Temperatures of around
20°C can be expected. We recommend that you bring an umbrella and a light coat for day/evening
time. Informal dress will be suitable for all workshop activities. |
| TRANSPORTATION |
| By Air Prague Ruzyně Airport, located 15 km from the city, is served by many international lines. The cost of a taxi from Ruzyně Airport to the city is at present about 20 - 25 EUR. Shuttle bus service operates as well. City bus No. 119 will take you from the airport to the metro line "A" terminal "Dejvicka". By Train Prague can easily be reached by rail. If you arrive by train, you will find a metro station on line C at the Central Railway Station or at the Holešovice Railway Station. City Transportation Prague has a comprehensive network of city transportation consisting of metro, trams and buses. Single tickets can be purchased at most newspaper stands and the coin machines at metro stations. |
| CAR RENTAL |
| Most of the major car rental companies have their offices in Prague. Detailed information is available
at the Workshop secretariat. We strictly recommend making reservations in advance. |
| OFFICIAL LETTER OF INVITATION |
| An official letter of invitation can be sent upon request. It must be understood that such a letter cannot
grant any financial support. |
| VISAS |
| Participants from the most European countries and the USA can enter the Czech Republic without a
visa. Other participants are advised to check requirements at the nearest Embassy or Consulate of the
Czech Republic and to make their own arrangements. Detailed information can be found on
www.mzv.cz under English version, link "Travel and Living Abroad". |
| INSURANCE |
| The Organizing committee will not accept any liability for personal injuries or loss or damage of
property belonging to workshop participants. Kindly check your personal insurance. |
| ELECTRICITY |
| In the Czech Republic, the electricity supply is 220 V, 50 Hz. |
| SOCIAL EVENTS |
| Friday, September 8, 2006: Closing party 19.30 – 23.00Hotel Morava, Jevíčko Included in the registration fee of registered participants. |
| ACCOMMODATION |
| Accommodation for participants has been reserved at hotels of several categories.
Kindly note that all capacities are limited and all requests will be
considered in order of receipt. We kindly ask you to indicate your second choice in the accommodation
form. Requests received after July 15, 2006 will be confirmed only within remaining possibilities. |
| Reservation |
| To book accommodation, a completed accommodation form should be sent by mail or by fax
to the organizing secretariat. On-line reservation is also available.
Inscription of your credit card data through the internet is SSL certified (Thawte certificate). |
| Hotel Deposits |
| Reservation will be guaranteed only after receipt of the first night deposit. The balance of the hotel
account will have been settled by July 15, 2006. |
| Cancellation Fees |
| Any cancellation must be notified in writing to the organizing secretariat. The payment of
accommodation will be refunded in accordance with the following cancellation fees:
Cancellation within 59 – 30 days prior to arrival: 50 % of the whole booking value
Cancellation within 29 – 15 days prior to arrival: 80 % of the whole booking value
Cancellation within 14 – 3 days prior to arrival: 90 % of the whole booking value
Cancellation within 2 – 0 days prior to arrival: 100% of the whole booking value |
| Confirmation of Reservation |
| Your booking will be confirmed upon the receipt of your Accommodation form and the hotel deposit.
The hotel voucher will be sent by airmail after receipt of the total amount. Reservation changes should
be made directly to the organizing secretariat and NOT to the hotels. |
| Prices and VAT |
| The prices indicated in the Accommodation form are based on Czech Crowns (CZK) and are per one
room and one night, including breakfast, VAT and city fees. In case of exchange rate changes by more
than 5% Conference Partners Prague Ltd. has the right to modify prices. FOR EUROPEAN UNION ONLY! Participants are obliged to indicate in the Accommodation and Tours forms the VAT number if the payment is made by a company or institution. |
